The ASI Order Form can be used to place large order with 20 or more recipients by simply e-mailing or faxing your completed order to Giftprogram.com.
Please indicate contact person's name, phone number and e-mail address, as missing information will delay your order. We will contact you for your payment information; please DO NOT include this in your email.
When an e-mail address is supplied, we will e-mail an order confirmation to you. All other confirmations will occur via phone.
Please do not send orders twice, we cannot be responsible for duplicate orders.
All orders are promptly processed within 24 hours!

Chocolate in our gifts may be replaced with other gourmet item of equal or greater value
from May through September, when the weather gets warmer.
These substitutions are done to ensure the timely arrival and freshness of your gifts
as well as the integrity of the presentation.
Do you offer shipping insurance?
All gifts are insured against loss and damage during shipping, at no extra cost to you!
Can I ship my gift anywhere?
We can ship to addresses in the 50 United States. we cannot ship to military APO-FPO boxes or foreign addresses,
due to customs and paperwork that would prevent the gifts from arriving within a reasonable time frame.
Why do you need my recipient's phone number?
UPS must have the recipient's phone number to qualify for our guaranteed delivery service. We will not contact
your gift recipients via phone, unless we are having problems with delivery.
How will I know when my order has shipped?
You will receive a shipment confirmation email after your gifts have been shipped. You can track delivery status
on our website; so be sure to include an email address and password when you place your order.
